Over investigated, over treated has been the lament of Indian patients. Doctors blame patients ‘They ask for it’. Patients are losing faith in medicos at large.
Often seen is a case given Statins for high LDL cholesterol with NO counseling on life style change/ physical exercise. Without blaming others, let us clean our own act!

Systemic corticosteroids are recommended for patients admitted with AECOPD, with benefits including shortened recovery time, improved lung function and hypoxemia, prolonged time to subsequent exacerbation, and reduced rates of treatment failure and hospital length of stay. It is as prevalent (if not more) in India as in western world.

Being a condition, which has no diagnostic tests except to rule out organic pathology; diagnosis is purely clinical; new guidelines will immensely help physicians especially primary care practitioners.

We all know that as high 40% victims of 1st STEMI succumb within 1st hour. How the patient is managed is of paramount importance. More important for primary care practice is prevention of such a terminal event and following new guidelines definitely help!

90-95% yield in Vit D assay; Another field where primary care practitioner has to look for vague symptoms eg minor aches and pains all over; some bone pains, discomfort in virtually any posture and ask for an assay.
Deficiency will be on the rise largely due to lesser consumption of milk/ milk products, less exposure to direct sun light and avoiding egg yolk, by all and sundry, on fear of cholesterol (consuming all other junk foods).
