With rising number of Diabetes patients all over world and especially in India, we need to establish a system which can have maximum access, to the patients. Since it is a life long condition it requires proper management. Thus we can pick up high risk patients at an early stage and prevent dreadful complications and can reduce burden on health care system.                                                                        Team@CMHF.

At Medical Specialists, a unique M3+ program is helping patients with diabetes manage their condition with easier, faster access to health care coordinators, information, medications and supplies, and answers to day-to-day questions and concerns.

Medical Specialists Department of Endocrinology is an all-inclusive diabetes treatment center, designed to better evaluate patients’ disease progression and control diabetic complications. Its exclusive M3+ program is an innovative disease management membership for those recently diagnosed with diabetes or who have difficulty maintaining glycemic control. It does not take the place of the patient’s existing medical providers, but offers enhanced care and maximum access to services that help manage diabetes.

A unique feature is access to the M3+ Health Care Coordinator. Members have direct access to the Health Care Coordinator by a direct dial phone number or email for questions and concerns, bypassing an automated phone system.

Education is key: Advanced diabetes education classes, interactive “members only” web site and a wide variety of on line tools are offered to M3+ members.

Better access is provided to diabetic medications and supplies through the patient’s pharmacy or Medical Specialists’ convenient on-site pharmacy.
